Background of the Study

Cancer pain is one of the most common and debilitating symptoms faced by cancer patients. The management of cancer pain is a fundamental aspect of oncology care, yet many patients continue to suffer from inadequately managed pain, which can significantly impact their quality of life. Pain can result from the cancer itself or from the side effects of treatments such as surgery, chemotherapy, and radiation. Effective pain management requires an individualized, multi-disciplinary approach, incorporating pharmacological and non-pharmacological strategies.

In Nigerian hospitals, including the University of Calabar Teaching Hospital (UCTH), the management of cancer pain presents significant challenges. These challenges are often related to limited access to opioids, inadequate healthcare infrastructure, insufficient training of healthcare providers, and patients’ cultural and social perceptions of pain management. Many cancer patients in Nigeria are also diagnosed at advanced stages, making pain management an urgent issue. Additionally, the stigma surrounding opioid use and concerns about addiction can complicate the administration of effective pain relief.

This study aims to explore the challenges involved in cancer pain management at University of Calabar Teaching Hospital, Cross River State. The research will focus on the barriers faced by healthcare providers and patients in managing cancer pain, as well as the strategies employed to overcome these challenges. By identifying these challenges and understanding the factors that contribute to inadequate pain management, this study seeks to provide insights that can improve cancer pain management practices in Nigerian hospitals.

Statement of the Problem (300 words)

Cancer pain is a common and distressing symptom that significantly affects the quality of life of cancer patients. Despite advances in pain management techniques globally, many cancer patients in Nigeria still experience severe pain due to inadequate management strategies. At University of Calabar Teaching Hospital (UCTH), cancer pain management remains a major challenge due to various factors, including limited access to pain-relieving medications (especially opioids), lack of adequate training for healthcare providers, and socio-cultural barriers to pain management.

The challenges in cancer pain management at UCTH are compounded by the late presentation of many cancer patients, who often arrive at advanced stages of the disease, making pain management more complex. The lack of standardized protocols for pain assessment and management, along with cultural taboos surrounding the use of opioids, has resulted in inadequate pain control for many cancer patients.

This study seeks to investigate the challenges of cancer pain management at University of Calabar Teaching Hospital and to identify strategies that can improve the quality of care for cancer patients suffering from pain. By addressing these challenges, the study aims to contribute to better pain management practices in Nigerian hospitals, ultimately enhancing patient comfort and quality of life.

Objectives of the Study

1. To examine the challenges faced by healthcare providers in managing cancer pain at University of Calabar Teaching Hospital, Cross River State.

2. To explore the impact of socio-cultural factors on cancer pain management in Nigerian hospitals.

3. To assess the strategies employed to overcome the challenges of cancer pain management at University of Calabar Teaching Hospital.

Research Questions

1. What are the main challenges faced by healthcare providers in managing cancer pain at University of Calabar Teaching Hospital?

2. How do socio-cultural factors influence cancer pain management in Nigerian hospitals?

3. What strategies have been implemented to address the challenges of cancer pain management at University of Calabar Teaching Hospital?

Research Hypotheses

1. H₀: Healthcare providers at University of Calabar Teaching Hospital do not face significant challenges in managing cancer pain.

2. H₀: Socio-cultural factors do not significantly impact cancer pain management in Nigerian hospitals.

3. H₀: Strategies implemented to address cancer pain management challenges at University of Calabar Teaching Hospital do not improve pain control.

Scope and Limitations of the Study

This study will focus on cancer pain management at University of Calabar Teaching Hospital. Limitations may include variations in healthcare provider practices, patient perceptions of pain management, and the availability of resources for effective pain control.

Definitions of Terms

• Cancer Pain: Pain caused by cancer, either due to the disease itself or as a result of treatments like surgery, chemotherapy, or radiation.

• Pain Management: The process of alleviating or reducing pain using pharmacological and non-pharmacological methods, including opioids and other analgesics.

• Socio-Cultural Factors: Cultural beliefs, social attitudes, and community perceptions that may influence the approach to and acceptance of pain management strategies, including the use of opioids.
